How do I change my life insurance beneficiary?

You will need to complete a new Beneficiary Designation form. Please contact the Benefits team by calling 239-533-2245 to get the form.

Complete the information in the APPLICANT, BENEFICIARY, CHANGE, and SIGNATURE sections of the form, and forward to HR Benefits. 

Please Note: Make sure you include the NAME, ADDRESS, and SOCIAL SECURITY NUMBER for each beneficiary you are adding or changing, in addition to the percentage you would like for each to receive.
